The following resources are useful to share with patients with intellectual disability and/or their carers
My Health Matters
My Health Matters is an Easy Read folder made to improve communication between patients with intellectual disability and their health care providers. It is filled out by the patient to share important information about the patients needs or concerns.
Health Passport
Julian’s Key Health Passport is a tool to improve care while in hospital. It is filled out by patients with intellectual disability about communication and potential adjustments needed while in hospital.
Health Action Plan
The health action plan is an action plan patients can go through at the end of their appointment. It covers important things about what the doctor shared, medication and referrals. This helps patients to remember what was said.
Ask Health Diary
The Ask Health Diary is for patients with intellectual disability. It is a physical diary filled out by patients, designed to give a sense of ownership over personal and health information. Sections include All about me, health advocacy tips, medical records and a section for the doctor (with ideas for clinical staff).
